Studies consistently show that the vast majority of enterprise AI initiatives fail to deliver meaningful business value, not because the models don’t work, but because organizations struggle with integration, workflow redesign, and human adoption. That redesign gap is where failures emerge.

Gartner’s Human–AI Axis: A Useful Lens — If You Finish the Job

  • How much autonomy AI is given

This produces four collaboration scenarios. Gartner’s intent is pragmatic: leaders must prepare for multiple futures, not bet on a single outcome. But there is an unspoken implication inside this model:

Each quadrant represents a different human risk and resilience challenge.

The Four Quadrants — Reframed as Missions for Human Risk Management

Rather than treating Gartner’s scenarios as forecasts, we can also treat them as missions — each requiring different controls, behaviors, and leadership attention.

Quadrant 1: AI Does the Work, Humans Handle the Exceptions

Mission: Preserve accountability when work becomes invisible.

In this model, AI executes most tasks. Humans intervene only when something breaks. Therefore the risk is not automation — it is opacity.

When AI becomes the primary actor, organizations lose:

  • Clear decision ownership

  • Situational awareness

  • Human intuition built through experience

Human Risk Management Teams and CISOs must lead by:

  • Defining escalation and exception-handling norms

  • Preserving investigative and judgment skills

  • Ensuring humans can explain, not just intervene

Exception handling is not a fallback — it’s a core human control.


Quadrant 2: Humans Use AI to Do the Same Work, Faster

Mission: Normalize safe human–AI collaboration.

This is the most common current state. AI augments existing roles without fundamentally changing job descriptions. It feels safe — but, as Shakespeare warned in Macbeth( Act 3, Scene 5 for those reading along), “Security is mortals’ chiefest enemy.” The goddess Hecate is warning that overconfidence, or a false sense of security, is the most dangerous flaw for humans, as it leads to reckless behavior, arrogance, and ultimately, destruction.  Comfort here is deceptive. Here's why: 

Risks emerge quietly:

  • Over-trust and under-challenge of AI outputs

  • Inconsistent usage norms across teams

  • Shadow AI and uncontrolled data exposure

CISOs and Human Risk Manager's role here is not training people how to prompt — it is designing and instilling the new norms:

  • What good use looks like by role

  • When verification is required

  • How confidence and doubt are expressed safely

Productivity gains without shared norms create distributed risk.


Quadrant 3: Humans and AI Redesign Work Together

Mission: Enable innovation without eroding trust.

Here, work is genuinely transformed. Humans and AI collaborate creatively, strategically, and iteratively.

This is where competitive advantage is created — and where risk accelerates fastest.

It is also where success is statistically rare. Research from MIT Sloan Management Review shows that fewer than 10% of enterprise AI initiatives successfully scale beyond pilots into sustained value creation, with most stalling due to organizational, workflow, and adoption failures rather than technical limitations (MIT Sloan Management Review: https://sloanreview.mit.edu). Forrester analysis similarly finds that only 10–15% of AI pilots ever reach durable, production-scale impact, while McKinsey reports that although AI usage is widespread, fewer than 40% of organizations see meaningful enterprise-level financial results.

In other words, this quadrant represents aspiration for many — but arrival for very few.

Key failure modes include:

  • Intellectual property leakage

  • Cultural drift between teams

  • Decision-making speed outpacing governance

This is where Human Risk Management must create a culture and a set of behaviors which becomes a resilience system:

  • Psychological safety to challenge AI

  • Clear ownership of outcomes

  • Embedded risk awareness inside creative workflows

Innovation without resilience scales mistakes, not value.


Quadrant 4: AI-First or Autonomous Operations

Mission: Keep responsibility human, even when execution is not.

In AI-first environments, humans design, oversee, and audit — but do not execute.

For most organizations, this remains a "city on the hill" scenario: aspirational, distant, and unlikely to be reached wholesale in the near term. Yet it is precisely because it feels out of reach that Human Risk Management teams should be considering it now — not as a destination, but as a future‑proofing lens.

This quadrant is most useful for long‑term planning, scenario modeling, and asking hard "what if" questions: Where might parts of the organization want to go? Which functions, teams, or workflows are already drifting in this direction? And what breaks if they do?

Against that backdrop, the existential question becomes simple and uncomfortable:

Who is accountable when no one touched the work?

This is where HRM moves from support function to governance backbone:

  • Defining human accountability boundaries

  • Preserving organizational memory and skills

  • Preventing hollowing-out and dependency traps

Autonomy without human accountability is not efficiency — it’s fragility.


Where Existing Frameworks Help — and Where They Stop

NIST AI Risk Management Framework

The NIST AI RMF provides an essential structure for identifying, measuring, and managing AI risk. It is intentionally flexible and non-prescriptive.

But that flexibility assumes something critical: that organizations know how humans actually interact with AI in real work. (A reasonable assumption in theory. In practice? Not what we tend to see. Most organizations love the ideal — and then quietly discover reality is messier, slower, and far more human.) 

NIST defines what must be governed. Human–AI work design defines how governance becomes real.

Without explicit attention to roles, behaviors, escalation, and judgment, NIST risks remaining theoretical.

MIT, Forrester, and the Augmentation Debate

Before we go any further, it’s worth pausing and asking: what do some of the smartest, most rigorous researchers actually say about this? The answer is refreshingly pragmatic — and far less starry‑eyed than much of the hype.

MIT research emphasizes that AI’s impact depends less on the technology itself and more on whether AI augments or automates human labor — and how deliberately that transition is managed. MIT Sloan Management Review has repeatedly shown that productivity gains emerge only when AI is embedded into redesigned workflows, supported by new decision rights, incentives, and human judgment, rather than layered onto existing processes (see MIT Sloan Management Review’s research on augmentation vs. automation: https://sloanreview.mit.edu).

Forrester reaches a similar conclusion from a different angle, predicting job reshaping rather than wholesale elimination, with many roles fragmenting into new combinations of human judgment, AI-supported execution, and oversight. Their research highlights that value creation depends on redesigning roles and responsibilities — not simply deploying tools.

Both are correct — and incomplete.

Augmentation without work design still produces risk:

  • Deskilling

  • Experience starvation

  • Overreliance

The missing variable is not technology. It is organizational design.


Why Human Risk Management Must Lead

AI workforce transformation collapses traditional boundaries — and in doing so, it is quietly reshaping the role of Human Risk Management.

For years, HRM (and it's former role, cybersecurity awareness training) has been treated as a peripheral function: compliance training, phishing simulations, annual acceptable use policy refreshers. Important, but rarely strategic. Often quite literally kept in the corner.

That era is ending.

As AI changes how work gets done, how decisions are made, and how accountability flows, the profile of Human Risk Management is rising — from awareness and phishing simulation into real human risk management and cyber culture operations. This is no longer about telling people what not to click. It is about designing how humans, technology, and risk interact across the organization.

What the Best Organizations Do Next

The most mature organizations are not only asking: “Which AI tools should we deploy?”

They are also looking deliberately over the horizon — beyond immediate use cases — to define transformational direction and acceleration strategies that will matter over the next three to five years.

They are asking:

  • How does work change here?

  • Where does judgment sit?

  • What could fail silently?
